By accessing or using the LIFETAG website (getlifetag.co), mobile dashboard, emergency QR cards, or associated services (collectively, the "Platform"), you agree to be bound by these Terms of Service ("Terms") and our Privacy Policy.
If you do not agree to these Terms, you may not register, access, or utilize the LIFETAG services.
2. Description of Services
LIFETAG provides a digital Emergency Medical Record (EMR) accessibility platform. Our platform enables users to enter medical history, emergency contacts, vitals, allergies, and supporting documents, generating a scannable QR code and web profile intended for emergency situations.
3. Crucial Medical Disclaimer & Emergency Notice
Important Notice: LIFETAG is Not a 911/Emergency Dispatch Center
LIFETAG is a record-sharing platform only. We do not dispatch emergency personnel, ambulances, or medical services. In the event of an active medical emergency, always immediately dial your local emergency services (e.g. 911, 112, or 999).
Furthermore:
- LIFETAG does not provide medical advice, diagnosis, or clinical treatment recommendations.
- We do not guarantee that third-party emergency responders, paramedics, or healthcare facilities will have compatible devices or choose to scan your QR code during an incident.
- The information displayed on your emergency card reflects solely what you have inputted into your profile.
4. Account Registration & Data Accuracy
To use LIFETAG, you must provide accurate, current, and complete information during registration. You are solely responsible for:
- Ensuring that all medical data (such as blood type, severe allergies, and emergency phone numbers) is kept accurate and updated.
- Maintaining the confidentiality of your account login credentials.
- Promptly notifying us if you suspect any unauthorized access to your account.
5. QR Card Usage & Public Display
Your LIFETAG physical QR card or sticker is designed to be publicly scannable without password friction during emergencies. By displaying or wearing a LIFETAG QR card, you acknowledge that anyone who physical scans the code will be able to view the specific fields you designated as public in your profile privacy settings.
6. Subscriptions, Payments & Cancellations
LIFETAG offers both Free profiles and Premium paid subscription tiers:
- Free Tier: Includes core emergency QR profile access and basic responder details at zero initial cost.
- Paid Subscriptions: Billed on a monthly or annual recurring basis as specified at checkout.
- Automatic Renewal: Paid plans renew automatically unless cancelled prior to the end of the current billing cycle via your account settings.
- Refund Policy: Subscription fees are generally non-refundable except where mandated by local consumer protection laws.
7. Intellectual Property
The LIFETAG brand, logo, platform software, UI design, text, graphics, and proprietary QR encoding routines are the exclusive intellectual property of LIFETAG EMR Technologies Inc. You may not reproduce, reverse engineer, or redistribute platform components without express written permission.
8. Limitation of Liability
To the maximum extent permitted by applicable law, LIFETAG and its officers, employees, and suppliers shall not be liable for any indirect, incidental, special, consequential, or punitive damages—including loss of data, personal injury, or failure of third parties to scan or follow QR records.
Our total cumulative liability for any claim arising out of these Terms or the Platform shall not exceed the total amount paid by you to LIFETAG in the preceding 12 months.
9. Account Termination
You may terminate your account at any time by selecting "Delete Account" in your account dashboard. LIFETAG reserves the right to suspend or terminate accounts that violate these Terms or engage in illegal or fraudulent behavior.
10. Governing Law & Contact
These Terms shall be governed by and construed in accordance with applicable state and federal laws. Any dispute arising under these Terms shall be resolved in competent jurisdiction courts.
